All the cane handles based on bycycle handlebars are doomed to have the foam tear and die. Original handle design was a heavy polyeurethane "cotton reel" and although never needing replacement, was exceedingly uncomfortable for extended use. This cane unlike many (any?) others I've seen use a sleek trilobular cross-section which, as it has a lot of handle bore - handle shaft area, is virtually twist free!
