Shields skin from direct contact with plastic bag, you can put in a washcloth for more padding if you want more. It has worked great for me, much more comfortable than the straps and no sliding down.

Answer:
It depends on the size of the child's leg. My husband used medium & large. I think medium worked best. It should not be difficult to take it in if the smallest is too large. There is a seam that runs down the exterior. This worked much better that straps for my husband.

Answer:
My mother's leg is about the same size. I purchase the medium size for her and sew a new seam about 1 inch from the sewn side, then cut off the excess. It only takes a few minutes.She's been using them for about 4 years and they work great. Hope this helps!
